Cortez High School is offering FREE physicals at Sunnyslope High School on April 14th.

ALL students must be transported FROM Cortez.  Physical forms are available now in the CHS bookstore. For more info please call 623-915-8246.

Please Note: All forms must be completed and signed by a parent or legal guardian in order to have the physical performed. All students are advised to wear shorts/T-shirt for the exam, while girls are advised to also wear a bathing-suit top.
