Athletic Eligibility
 
To be eligible to participate in interscholastic activities the following guidelines must be met:
 
     1.   Students in their first through sixth semester (Freshman through Junior year), must be
           enrolled in a minimum of five (5) credit earning classes.
 
     2.   Students in their seventh and eighth semester (senior year), must be enrolled in a
           minimum of four (4) credit earning classes.
 
     3.   Students shall be required to earn passing grades in classes which they are enrolled.
 
Students will be monitored weekly for eligibility.  Students who have failed to earn a cumulative 
passing grade as established by a teacher at the end of any designated interval must bring up
his/her grade with the appropriate teacher/teachers who will verify passing status with the
athletic director before becoming eligible to play again.
 
In addition to being scholastically eligible, athletic participants must have the following on
file in the athletic office:
 
     1.  Cortez Athletic Physical card which includes:
 
          a.  Physical exam information.
          b.  Parent/Guardian consent form.
          c.  Medical emergency information.
          d.  Proof of medical insurance.
     
     2.   Informed consent card verifying the student has viewed the At Risk Video.
 
     3.   Record of certified birth certificate.
 
     4.   Student must be issued a permit to practice by the athletic office before they are
           allowed to practice or try out.
 
You may pick up a physical card from the bookstore or reception area.  If you have access to a computer you may print a physical form by clicking UserFiles/Servers/Server_759803/File/PhysicalForm.pdf   Complete the remainder of the form and have your athlete return the form to the bookstore.
 
Each April, the Glendale Union High School District sponsor the "FREE PHYSICAL" day.  In order to
participate the parent/guardian must complete permission/consent forms for their
student/athlete to have the physical.  School bus transportation is provided to and from Cortez. 
Incoming Freshman are also invited to attend.  Pick up consent form/physical card
in the reception area in early April, 2010
